Which Occupational forms require fingerprints?

Current forms that require Fingerprints:

  • Colorado - Gaming and Sports Betting Support License

  • Illinois - Occupational Level 2 and 3

  • Indiana - Personal Disclosure Form 2

  • Michigan - Occupational License Application

  • New Jersey - Personal History Disclosure Form 3

  • Ohio - Standard Sports Gaming Employee License Application

  • Pennsylvania - Gaming Permit

  • West Virginia - iGaming Occupational License Application

Please double-check all forms to make sure fingerprints are required.

Please note that some states may accept digital fingerprint options but most require wet ink fingerprints. Please check with your state regulator to rules and instructions on the type of fingerprints they accept.
